NSW Perinatal Data Collection form
Sample copy of the one sheet data collection form completed by the attending midwife or doctor when a birth occurs in New South Wales, Australia.
The form includes demographic items and items on maternal health, the pregnancy, labour, birth, and perinatal outcomes. Completed forms are sent to the Health System Information and Performance Reporting Branch within the System Purchasing and Performance Division of the NSW Ministry of Health, where they are compiled into the Perinatal Data Collection database.
